Volunteer Hours for Graduates
Students should check with their guidance counsellor and/or principal to see what will count towards their volunteer hours for this school year.
Please submit your Community Service Hours only when you have completed ALL 40 hours.

- Community Involvement Student Passport
Both the Student Passport and Community service hours form need to be submitted to the Guidance office.

How do I request a Student Transcript required for post-secondary institutions?
For current Holy Name students
Fill in the Transcript Request Form below and email it to hnmryguidance@dpcdsb.org
For students already graduated (not current school year)
Send Transcript and Diploma Request inquiries to transcripts@dpcdsb.org
